Minimalist Mirror Reflection

If you’d like to learn more about any of our services,

please get in touch—we’re happy to talk you through

what best suits your needs.

At Social Bloom Consulting, we believe that support and advocacy should feel accessible, compassionate, and centred around the individual.

Whether navigating disability, ageing, health changes,

injury, or complex life circumstances, we are here to provide thoughtful and professional

support every step of the way.

Social Bloom Co is a welcoming and inclusive

space for people of all ages, cultural backgrounds,

abilities, and lived experiences.

We value each person’s story and are committed

to providing respectful, person-centred services

grounded in empathy and social work values.

Professional assessments can still feel personal, compassionate, and supportive

Assessments are completed from a psychosocial and person-centred framework, exploring the psychological, emotional, social, environmental, and functional factors impacting wellbeing, participation, independence, and daily life.

Reports are completed within the professional scope of social work practice and may differ from assessments provided by other allied health disciplines.

Out Approach Includes:

  • a holistic approach that considers each person’s lived experience, environment, strengths, and support needs.

  • Work collaboratively with clients to ensure their goals, experiences, and voice remain central throughout the assessment process.

  • Provide compassionate, professional, and evidence-based assessments and reporting.

  • Recognise the complex ways health, disability, mental health, trauma, and environmental factors can impact wellbeing and everyday functioning.

  • Support individuals to access appropriate services, funding, and supports through clear, thoughtful, and comprehensive reporting.

Our Practice Values

Social Bloom Consulting is grounded in compassion, authenticity, social justice, and respect

for human dignity.

Guided by feminist, trauma-informed, and relational social work perspectives, we value

  • safety

  • collaboration

  • cultural humility

We believe every persons story deserves to be approached with care and without judgement.

Our service is person-centred, strengths-based, and trauma-informed. We focus on understanding each individual within the context of their life, relationships, and environment, rather than viewing concerns in isolation.

We work alongside children, young people, adults, and families, as well as those who support them. This includes support coordinators, advocates, and other professionals involved in a person’s care and wellbeing.

Minimalist Mirror Reflection

The engagement process is designed to be clear, respectful, and supportive. This involves:

  • An initial referral or contact.

  • Gathering relevant information through conversation and collaboration.

  • Preparation of a clear and thoughtful report tailored to its purpose.

  • Ongoing communication where required.

  • A strengths-based assessment process. Where appropriate, we incorporate recognized assessment tools to support a clear and well-rounded understanding of each person’s strengths, needs, and circumstances. These tools are used alongside professional judgement, lived context, and collaboration with individuals, families, and those supporting them to ensure a holistic and meaningful understanding.

Assessment tools may include:

  • Vineland Adaptive Behavior Scales (Vineland-3)

  • Strengths and Difficulties Questionnaire (SDQ)

  • Kessler Psychological Distress Scale (K10)

  • Child Behaviour Checklist (CBCL)

  • Parenting Stress Index (PSI)

  • WHODAS (World Health Organization Disability Assessment Schedule.

Throughout this process, care is taken to ensure people feel informed, respected, and included.Where appropriate, we also collaborate with services to ensure a more holistic and well-rounded understanding of each situation.
